Insurance Information

Appointments
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, and Visa for your convenience. If you have a health plan with copayments, they are payable at time of visit. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
Prescriptions & Renewals
If you have a prescription that needs to be refilled, please call the pharmacy that last filled your medication at least 24 hours before you will need the medication. If you have a new prescription need, please call our office during business hours. Again, be certain to allow at least 24 hours to make any arrangements for prescriptions.
What To Bring To Your Visit
 Your insurance card or other health information.
Some insurance carriers require you have a referral or primary care authorization before treatment is given. Please check with your carrier.
X-Rays, MRI’s and/or reports if available.
Privacy Notice
Proliance Surgeons respects your privacy. We understand that your personal health information is very sensitive. We will not disclose your information to others unless you tell us to do so, or unless the law authorizes or requires us to do so.
The NOTICE OF PRIVACY PRACTICES describes in more detail how your health information may be used and disclosed, and how you can access your medical information. When you visit us, you will be asked to sign an acknowledgement that you have been informed of the Notice of Privacy Practices.
